ARE THE PAPER DATA COLLECTION FORMS DEVELOPED BY ICADV MANDATORY?

No. Program administrators may revise these forms or recreate their own, as long as the required information is collected. ICADV-created intake forms include those for adults and children and for residential and non-residential programs; medical criminal justice process forms; service note documentation forms for adults and children; hotline call log form; turn away data collection form; group services form; and community and institutional data collection form. These forms may be requested by contacting CJA.InfonetHelp@illinois.gov.
